Guys, Leave a note or not?

I have a gym crush who I believe might be interested in me as well (I caught him looking at me a few times). I go on my lunch break so I'm always in a rush and don't have the time (or courage) to start a conversation. We have arrived at the same time a couple of times so I know what car he drives. Would it creepy to leave a note with my phone number? Something like "I have trying to say hi but don't feel brave enough - if you're single here is my phone number" Or any other suggestion?

Arrested Development, if you haven't seen the series; it is worth watching the first 3 seasons. The 4th season, the Netflix reboot, wasn't as good; but did have its moments.
