Rarely and never without very explicit go-aheads.

Basically if we have not been obviously staring and giggling at each other or in some other very obvious way shown mutual interest I simply won't approach.

Which is why I like to see it as women often taking the first step by inviting the guy. Or I suppose whoever stares too long is making the first move.

I can't speak for men, but rarely am I approached unless I give some acknowledgement to the man that I noticed him. If I make eye contact and smile, they may approach.

I do approach women I find pretty, but usually I'm the one getting approached. I base my approach on what the woman is like. I usually get to know her first and then know what she likes.
