It sounds like a sweet gesture, and a simple, anonymous valentine is cute, but you might want to rethink where you put it. A note can simply be slipped into someone's locker, but a rose makes it obvious that you know the combination.
If you're a friend seeking to reveal your crush and you didn't get the combination in a suspicious way, you could go ahead with your plan and leave a little hint pointing to you.
If you're admiring from afar, do something less invasive. Ask the office to deliver your gift, or place it on your crush's desk before the person gets to class, and don't forget to put the name clearly on the valentine so that it's obvious who the intended recipient is.
It depends. If she TOLD you their combination and you know each other, it's probably pretty cute and if you put a message saying you like her, you're on your way to being a couple. But if she didn't tell you, and/or you don't really know each other much, then it's probably creepy.
That's cute by all definitions dude. People labelling everything as creepy results in dudes second guessing themselves on stuff like this.

Sweet is a better description than cute. It is only creepy if the guy is stalking and I assume you are not.
WOW is all I can say to that one. She's a luckey girl. That's a lovely thing to do whether she knows you or not. She should appreciate it.
