What would factor into your decision of leaving a restaurant without paying the bill?

I wouldn't just walk out without paying. If there was a serious issue with the food or service I'd ask for the manager and let him deal with it. If the manager doesn't care to fix the situation then I'd consider not paying and I'd never come back. But things would have to be really bad.

I would never not pay the bill. If the food and/or service was horrendous, I would speak with the manager and expect them to comp st least part of the bill.

I could leave without paying if I didn't order anything but I would at least notify my waiter

I would never do that unless I had a real good faith dispute with the restaurant over the quality of food or service. And even then I wouldn’t sneak out. I would stalk proudly out the front door.
