There are all kinds of answers I could give and there are some good ones already listed like smoking which would apply for me as well, but I will go with certain kinds of food like shellfish and watermelon. I am allergic to them and wouldn't want to suffer consequences from trying them again.

Cigarettes. I smoked once, when I was 5, and my older sister (13yo) gave me a lit cigarette to show her friends how her little brother smoked. I hated it, and have not touched a cigarette in the decades since.
