The doctor did it using a too-large speculum when I was 18 during my first pelvic exam. It was like being raped. He shoved a big metal tool inside me and winched it open. It hurt like hell and my underwear was bloody. I went home and cried in the corner of my room.

I was in college in first year and was studying criminal law. We were learning the phonetic alphabet.
