When you say “she’s a lovely girl” what do you mean by it?

I think of it as a formal term denoting beauty. Like I told my friend she looked "lovely" because she and another girl were all dressed up and I didn't want to use more casual terms like "you looked hot" because we aren't in a relationship.

It's both. Lovely means they're lovable, beautiful, wonderful, all things good.
Unless it is said specifically about looks or personality.
It's just a compliment, a platitude. Don't put too much stock in it - guys will say ANYTHING to get in your knickers.
