It’s really bad , sometimes the pain isn’t like life ending if you get a slap but it never fails to make me collapse.
In mma actually all those super tough fighters are required to wear cups and they almost always collapse from it even with the cup and being some of the toughest guys there are who can tank punches to the face. I’ve also seen slaps take down guys before without cups.

This wrestler appears to have taken a light graze but his reaction shows that he is likely in pain from it and yes depending on the scenario a graze can hurt a lot!:

I grazed my balls against a door handle once while freeballing and it hurt more than some medium force slaps even. Sometimes it can hurt more although it depends on the guy.
Is it painful? Yes. But for some reason women think men just get completely incapacitated from a nut shot. Some will, especially if you manage to hit them square with a knee, but you may also just really, really piss them off. It’s not like they are just going to let you go if they are trying to sexually assault you. You’re much better off poking them in their eye. Take your finger and dig it in their eyeball past your knuckle. They’ll let you go. Will grab their eyes, and stand there and blink and clear their vision.
Just watch UFC. Guys just shake off nut shots. Sometimes they don’t even stop to take a break for an illegal groin strike. But an eye poke? God champions get poked in the eye and they HAVE to stop. They even talk about just how damaging and disconcerting a finger in the eye can be.
Yes, they are extremely sensitive. Thankfully I've never been hit really hard there, but even a light or moderate hit (I've accidently done it to myself) can be extremely painful. I could definitely see someone being physically sick and vomiting. We aren't exaggerating. Can cause real serious damage to him also. Personally while I do think it's a good "easy" way a woman can defend herself, just know you don't have to do it hard to be effective! lol
It hurts bad. I was once the goalkeeper, decided to defend against the attacker with using my stomach to stop the ball but miscalculation and it hit my balls. I fell on the ground, incapable of doing anything. I can say I nearly cried 🤣. After some adrenaline rushed in my blood, I got up and tried to continue the game but they didn't allow me (as I was considered injured)
It hurts bad and the worse it is the further it spreads. Like when I somehow missed catching the football when I was playing catch with a girl and it slammed right into there. It hurt bad and I was able to stay standing but it hurt for a few minutes and it was as if the area itself felt nauseous for a few hours. Just kind of achy
You can't breathe, you can't talk, you can't cry, you get these cramps in your stomach and you feel strange sensations all over your body, and all you can really do is just to wait for your senses to get back on track. It literally feels like you are dying. You have so many pain receptors flaring up at the same time, it doesn't even feel like pain anymore, it feels like death.
You can find out pretty easy just let someone hit you down there. It does the same pain to a woman if hit there.

It can be really really painful, probably the most painful thing a guy can experience, similar to childbirth in women. That area is very sensitive.
