He had a stroke possibly. Or a cerebral hemorrhage if it follows head trauma. Depending on where the stroke is localized will affect the degree of cognition and depth of awareness. Though not meant for this, the Glasgow Coma Scale can show diminished cognition of subjects not in a coma, and while a comparison of coma/non-coma subjects would not be appropriate (apples to oranges), both present in a diminished depth of cognitive ability when compared against a normal person's score (15).
