Nothing. They just ask you questions. They don't do pelvic or breast exams at MY doctor's office at least until you're 21 or unless you've asked for them.
I'm 19 and went to got tested for STIs once because I was having ovarian pain so she swabbed my cerxix and sent it to the lab, but even when I asked to get tested for STIs previous times they just did blood and urine samples. The only reason she swabbed my cervix was because I was having a really bad pain inside me.
The doctors shouldn't have really checked your sister's vaginas unless something was significantly wrong with them. Like if they were examining them for a rape case or they were bleeding for like 2 months straight when they were only 8 or something like that.
Usually, what happens when I go to the doctor is I sit down they ask me a few questions like "when was the first day of your last menstrual period?" and then they weigh me and take my blood pressure you know the usual. Then the doctor comes in asks me if I'm feeling a loonngg list of symptoms (headache, fatigue, fever, cold, cough, etc) and records that then we usually talk about what's wrong with me and I tell them and they give me the solution.
Just to let you know when I was 11 I had 2 periods and then NOTHING for like 9 months then my periods started back up again regularly. I talked to another friend of mine and she did the same thing so I think it's normal. I think what may be happening is you get a surge of hormones at once and then your body doesn't really know what to do with them (something similar might happen if you get on certain birth controls).
As for solutions the doctor is probably going to tell you it's normal or do blood work on you and maybe ask you if you want to get on the pill to even out your cycle. If you get on the pill they still don't need to examine you they just write you a prescription for it and give you directions on what you need to do and you go pick it up after your appointment

On the first year, it's really common to miss your periods. it can be two month to even six month. It takes time for your body to adjust to it.
Tell your mother that a generalist is enough for that, it would be only to reassure her.
You don't need to see a gynecologist unless you don't have your periods for six month or more, or if if doesn't regulate in a year or so.
In the first year, your period is not regular, it is completely normal. I had that too and I didn't even saw a doctor for that :) no need to worry
