It is a tiny bit of scar tissue with a cyst underneath.
This type of thing that does not go away without a minor surgical procedure because it is scar tissue. Underneath the skin is a tiny ball filled with dead skin cells. If you plucked it out, if would look like a ball filled with white paste (kind of like squeezing a whitehead).
If you want to get rid of it, go to a dermatologist who can anesthetize the area and cut it out in a way such that it will heal over properly. If you try to cut it out yourself, it could get infected or you may not remove it properly/completely.
Any such procedure would probably heal over in a week or so.

No idea what it is, but a regular margin, not discoloured, and by history not changing much.
Pretty sure it's harmless but if it bugs you see your doctor to get it treated. Do no do it yourself.
Looks like milia to me although the photo is a bit blurry. If you're that worried about it and it doesn't clear up within a few days I would recommend seeing an esthetician or dermatologist for further analysis
It's just a spot and will fade just keep on washin your face/toning/moisturising but it's odd that it's been there for a year now
