My friend got hit in the crotch and now one of his testicles is very swollen?

My friend was playing with his little sister and she jumped on him and ended up landing on his crotch weirdly. He is in a lot of pain and I convinced him to let me take a look (I did some medicine schooling) and his left testicle is very big and bruised. It hurts him very much when it is touched.

I think he should see a doctor but he is downplaying it despite his obvious pain. Am I overreacting, will it go back to normal or does he need medical attention? He won't let anyone else look because of how much it hurt when I did so no one else really knows whether it's that bad or no.

What I do if I'm injured is I try to sleep it off. Then if it is worse the next day I try to sleep it off again. If it continues after two or three weeks I go to the hospital. I remember one time I went and it's like my art hurts. Then the doctor asked me if my arm hurts when I do this, and I said, "yeah", and that, and I said, "yeah". Then they asked me to rate the pain and I said like 4.5/10. Then they took x-rays and my arm was broken. But you know, the spirit is stronger than the body. If he doesn't need a hospital then I don't think he needs it. Or maybe he needs it but he will die in a fighting way.
