both are bad, caffeine because it can increase your heart rate especially if you are sensitive to it, it is a stimulant, sugar raises levels of uric acid in the blood, which interferes with the production of nitric oxide which helps blood vessels flexibility, so when thats lowered it causes high blood pressure
I'm no doctor but I'd go with added sugar. Sugar is more addictive, and it's harder to avoid.
Probably sugar in the long term. And coffee is generally delivered in a safe dose, sugar should be regulated very hard, because it's such a serious problem that's slowly poisoning humanity.

Sugar is poison. Caffeine increases blood pressure only temporarily and even then if you are a regular consumer of caffeine then you become desensitised to even the temporary spike in blood pressure and that stops too.
However, I’m not telling you to fill yourself with coffee, just that caffeine’s effect on blood pressure is not as significant and in normal cases, caffeine even has many benefits.

Caffeine is at least nutritious in other areas. Sugar is just sugar
caffeine acts differently to sugars, caffeine basically stops your brakes being applied (it puts off tiredness/fatigue as opposed to giving a boost) and sugar does essentially the opposite
