It seems there is a broken bone, and the doctors tried to fix it but there still was a gap between the bones.

At least thats what i understand.

The break of the knee is starting to heal over but it still is distribution broken and there's swelling in the tissue around it

  The partial fracture cleft she's there is still some separation in the break and it hasn't fully bridged the gap of the break in the healing process

partial bridged is like partial healed of broken bones so there is still some gab left, it's like the bone is incomplete at the fracture point.

there has been some/partial healing around the fracture, though the fracture is not fully healed just yet

Not a medical opinion but the Patella is a Tendon attached to the bottom of the Kneecap to the top of the Tibia (shinbone)
I learned that in Nursing School but it could be something else?

It means that something is left in the knee. They want to remove it in intervals through the kneecap.
Tissue is swollen etc.

  The joint effusion and soft tissue is all you have to focus on. The medical term for the fracture in layman's terms is minor calcium deposits due to putting pressure i. e leaning on one side
