Sometimes bad breath is a dental issue.

I have a friend who had bad breath and had a fear of dentists. I had a great dentist that didn't hurt his patients. He went to see him, and the problem was dental issues.

After the dentist fixed all of his affected teeth, no more bad breath.

I'm glad he was open to the idea and took the appropriate action.

just take him to one to one side and mention it to him, ask if everything's okay.
a lot of depressed people for instance makes them neglect "simple" care such as teeth brushing.

No, that's rude. Bad breath sometimes can be a medical condition it doesn't necessarily mean someone has bad oral hygiene. Ask him if he's aware that he has bad breath.
