They are not necessarily unhygienic, or at least no moreso than any public toilet facility. Where they can contribute to a lack of hygiene is by making some guys feel that because they've not touched a toilet they don't have to wash their hands. They're fine with handling their cock and not washing their hands. If urinals didn't exist then guys would likely wash their hands more after using public facilities.

No idea if it's more or less hygienic. I would think it diffuses the urine into the air more and offers less privacy. I don't see them as frequently as I used to but whatever, I never use urinals anyway.

We don't pee from our vaginas. Our urethra opens and directs pee downward, not out away from the body. There are some women who pee standing up, but the urinal in the picture would still be hard to use.

There are female urinals as well, but very uncommon to see in this day and age.
