Well if you rewatch the first episode I think his first choice was Squirtle. Then he goes down each one to find out all of them were taken.

Then I think it would play out similar to what happened in the games. Gary will pick the opposite of what you picked and challenge you to a duel... I mean battle.

  Or the plot would be just like the games. Ash will turn into Gold in Johto, Brendan in Hoenn, Lucas in Sinnoh and so on. That's why those characters aren't in the anime. Same plot but with Ash's body.
