It is been YEARS since I've seen this show. I had to look it up because a lot of shows I'm having a hard time remembering now. But she didn't creep me out per se. I just thought she looked strange for an old woman designed as a puppet. The puppet's name was Lady Eliane Fairchilde. I think what it is is that her design comes off as creepy because many of us have been conditioned that people who often have those features come off as mean-spirited, beyond strict, and unreasonable. But it is also possible many of those puppets have spirits in them anyway. A lot of people in that industry do witchcraft, especially in children's programming.

The creepiest Mr. Rogers was when they did a special trying to talk to kids about murder and relate it to the Land of Make Believe. They had a one time character called "Walking Tree" who was supposed to be a puppet Tree Kid, who ends up getting murdered and its reported across Make Believe. At one point one of the adult humans is seen carrying his body as everyone is crying, and later they found out that he was killed by an adult "Walking Tree" whose only response to 'why did you kill him' was this goofy-voiced "I... I don't know!" It was bizarre.
Lady Elaine Fairchild. Nope, never thought of her as terrifying but, kids today are scared of their own shadows!! EVERYTHING is creepy, to them!! The only creepy thing I ever recall seeing on a kid's t. v. show was the grandfather clock on Captain Kangaroo.
I honestly found the whole "Mr Rogers" show hella creepy growing up. That tone of voice he would speak with always gave me 'rapist' vibes
I think Lady Elaine was modeled after a punch and judy puppet. One of them usually had a long red nose.
