I really like making notes daily with a few categories:
- to do (have to do)
- want to get done (can move back a day if needed)
- meetings/appointments
Then I'll make a schedule by hour, and start assigning time estimates to each task. Start with meetings, then prioritize to dos, then I fit in the things I want to get done.
Crossing them off and having an hour by hour breakdown is handy for me. It helps me keep focus and if I finish big tasks first I can fit in extra small tasks to keep busy.
The nature of ''my boss'' requires to only make a plan for half a day.
This plan is in my head - and I follow it up.
The other half of a workday is solving surprising issues that he had procrastinated (as usual).
I don't need a specific ''planner'' for that - any present project is a file on my 'desktop'; anything that's finalized but not finished has its own folder; and shit that's done or never will be followed up goes in the ''history'' folder.
I had a ''to-do'' (excel) file for myself before; but that one is history as well :)
One thought of being ''productive'' (as you had asked): ''Productivity'' is self specified. I can look productive all day - but achieve next to nothing.
The ''trick'' is: anticipation. Be prepared for what gets on the desk to handle it correctly, and with no unreasonable delay.
Start with your stuff that can't be moved (like meetings, work, school) and work backwards to see what you can fit in. That's what I usually do.
I have 3 types of checklist in my journal. One for morning, noon and night. That's the plan for the whole day of which I take time writing before sleep.

That sounds horrible. I mostly make a game plan in my head of things I'd like to get done, thats it. What you describe sounds way to ridgid. I like spontaneity so my game plan changes based on what the day brings. I like not knowing.
I love do it yourself journals but do not own one, maybe I could ask for one on my birthday.
Plan my day? That sounds horrible. I totally wing it and always have.
I like to write down step by step what ill do in a day then I rip the paper and say " Fuck it"
I just keep track on a monthly calendar by my computer.
A quick glance and I'm good to go.
if I wake up... (didn't die in my sleep) I get up and watch TV
I don’t have that kind of life where I plan to do things. Things just happen
I just go with what I feel. If I have to organize something, I'll put it on my calendar.
I just take each day as it comes.
Zero planning goes into my day. Lol
