It means pound weight, so if your weight was 120, it would be #120
it was later used in a general numerical sense, where "#10" would translate to "the number 10" this lead to it being added to phone dials, and simply referred to as pound, pound symbol, or number symbol.

I think in the US it is often called the pound sign for pounds weight but that is not used elsewhere. It would be written as 10 lb elsewhere not 10 # (if that is how it is indicated in the US).
It should be regarded as a part of the National Character Set which can be a different glyph in every country. # could be officially pound in the US but not anywhere else.
I would tend to think hash is the most common usage across the globe.
