I did a couple of times before because I had to LOL. I always carry sanitary wipes so that I can clean both the toilet seat, and myself right after :)

I make sure I don't have to. I am not too comfortable with the hygienic aspect of public toilets.

Depends: hospital? University? Sure. They're clean, but if I can help it, never gas station, grocery store, hardware store, etc.

I hate to, but if i have to go, i lay down toilet paper on the seat. bring hand sanitizer. If a room is nasty enough, i will go pee on a tree.

Not if It doesn't look clean. A good number of rest stops actually have clean restrooms. I prefer not to, because not all are.
