Depends on the test. I scored 104 in my TOEFL with minimal preparation recently. I tend to be confused in MCQ based tests which is kind of mathsy, I'm more comfortable with written answers or tests with more simulations like TOEFL.

Like I can comfortably take any integration test with difficult to difficult questions but I am absolutely ruined when taking SAT, not because it is hard but because so many MCQs bores me out easily and I'm fatigued early.

It depends. I get bored easily and tend to not take a test seriously if it's poorly constructed. At that point it just becomes fun manipulating the test results for maximum entertainment.
