Hard to say what that means definitively, but it isn't a good sign. He obviously still has feelings for his ex. How long ago did they break up? Honestly, this sounds like one of two things happened. Again, neither are good signs.

Either he meant to send that to his ex and accidentally sent it to you, indicating that not only does he still has feelings for her but is in communication with her as well. Possibly even seeing her on the side.

The second possibility that comes to mind is that he was intoxicated and sent this text to you when he wasn't thinking straight. I haven't experienced someone doing that to me, but it sounds like something someone that had too much to drink or something would do not knowing what they were actually doing.

No matter the case, you need to talk to him about that. It is not a good thing for him to be thinking of his ex as "sexy" while he is in a relationship with you. Hope this helps.
