It's paid in every country. There is no such thing as free. If the "government" pays it, they are only acting as a middle man between the people and health care. Either way the people pay it, because there isn't anyone else who CAN pay it.

If the health service is paid for by the tax payer then it's not a middle man, it's the provider. This is more common where the commercial sector hasn't taken more control of the government and made the provision of health care an industry.

No we in Pakistan don't pay for ambulance, there is one Charity Organization, EDDI WELFARE TRUST INTERNATIONAL , and Government Rescu Organization 1122 , providing free Ambulance Service all the time
