Probably something simple like no one in a group remembers something exactly, then it's natural to fill in the blanks with what sounds right, so they tell it to the group and with no one able to come up with a better explanation, they all cement it in their minds that it must be what happened. You picture what someone said in your imagination and if you repeat it enough and it seems plausible with no more definitive memory, it becomes fact to you. People either stick with they don't remember or they repeat it enough to believe it. Mandela was just herd mentality when someone started saying it and it couldn't be proven otherwise until he was released, it was "logical".

Depends if you believe in consciousness or dimensions.

If you believe in consciousness, then you know mind and memory can play tricks. So you might occasionally "recall" things that are not quite true.

But if you believe in dimensions, then you can fall into various conspiracy rabbit holes and think there are "rifts" where things change for some people and not others.
