Typically today, what you are buying is a "cat-back exhaust" meaning that it replaces everything behind the catalytic converter (pipes, muffler[s], and tips). Usually the old exhaust is cut off (or unbolted on some cars) and the new exhaust welded or bolted on in place. Typically, but not a8, the hangers are in the stock position so that it's easy to do that swap.
More serious performance builds include replacing the stock exhaust manifold with headers and even replacing the cat with a higher performance model, but that's not only a lot more difficult and expensive but also has legal ramifications (messing with the cat is a big no-no in many states).

Usually there are hangers and clamps that need to be replaced. The gaskets that go between the manifold and the tailpipe need to be replaces too.
