My friend told me this happened with an ex of hers, and I was like, I don't even know wth I would have said if that were me, because for me, it would make me think he was already cheating and now just needed permission to do so which even the thought, makes me want to vomit. Obviously she broke up with him. I'm like her, like, I didn't sign up for a three way call, ok!

Yeah, that's a massive red flag, for me, and I'd break up with them, for a couple reasons.

The first being that it's obviously something they've thought about, and want, which means they no longer want to remain in a monogamous relationship. That, alone, tells me the relationship is over, because we have fundamentally different values about what a relationship should be.

The second reason is that I'd no longer trust her. It's extremely likely that she already has someone in mind, and has either already cheated, or is basically asking for permission to do so. So, by all means, go fuck the other guy, but don't come back afterwards, 'cause we're done.

The third reason is that it would just hurt, emotionally, for someone to even ask me that. They're essentially saying that I'm not enough, and they want more. How am I supposed to continue in a relationship with this person, feeling like they don't respect me?

I've been with someone who suddenly told me he wanted an open relationship, that he was polyamorous and such.
We broke up and we got back together and it was overall the worst I've experienced.
Yes, he was manipulative and other stuff.. but trying to make it work with someone who has someone else... it nearly broke me.
