I am extraordinarily low maintenance. I'm just tickled that a dude shows up! The best dates are outdoors, and seeing how spring time is among us, go with that. Text her, say, don't wear your heels today babe. And show up in a white tshirt and some shorts. Take her to a walking or hiking trail and tell her how much more lovely she looks than the blooming trees. Then apologize for the cheap date by throwing some money down on a nice meal. And wear what you're wearing. Around where I live, in the mountains, there are so many nice waterfalls surrounded by outdoor restaurants. A budding couple could really get to know each other in this atmosphere. But I'll tell you, if she's worth pursuing, she won't care what you're wearing, she'll just be tickled that you showed up.

Depends on the person and occasion but i guess smart/cas. I personally prefer jeans (tight, so i get to see the goods lol) and then a smart shirt, can either be shirt with buttons or just a t-shirt, something fitted to your body is always good. Shoes wise - shoes or vans or something, not like workout trainers. But thats just me. Where are you going?
Yes, it does depend on where the date is and what is being done on the date.
If it's more of a typical date (movie, dinner, walking, concert, or something) then a nice shirt (t-shirt or button up) and jeans or slacks is fine. If it's something more active (like rock climbing, hiking, surfing, etc.) then something more appropriate for that would be better than something dressy. I think a nice shirt and pants work just fine for most things, though.I think it obviously depends on what you're going to be doing on the date but it depends on the person too. Personally, I'm not a very fancy person. I'm very casual and so I prefer a guy to dress casually too. I'm more attracted to a guy in jeans and T-shirts than a guy in a suit and tie. So this will definitely depend on the girl and her individual style as well.
Unless you're doing something very outdoorsy, I think a shirt is nice (like a buttoned one) rather than a tshirt. Jeans are ok as long as they're not too scruffy, or chinos. Depends on the time of day too I guess. Once I went on a date with a guy wearing a full on suit and tie. He had just been to a job interview so it was ok but I think it would have been a bit much if he hadn't just come from work
My boyfriend wore a collared shirt and some nice shorts on our unofficial first date... I was very impressed and felt like a slob in my t shirt and jean shorts. I made sure to dress up on our next, official first date XD
It depends on where you're going but, in my opinion, you can't go wrong wearing a semi casual button down shirt with a nice paid of jeans :)
