I once saw a girl at the bus stop who had very, very severe acne (EXTREME), 10 times worse then the picture above, the girl that I saw, her face was covered from forehead, nose, eyelids, around the mouth, cheeks, chin and I was completely in shock. I think it's safe to say that if we deal with 1 pimple we freak out, myself included.
I sympathize with people who have had acne issues, I don't know what the case may be? Is it genetic? Is it what you eat? Lack of exercise? Lack of drinking water? Using make-up I don't know but it's not cool to live like that and always have your self-esteem below when you go out in public or just even in the comfort of your own home.
My only advice is 1) speak to a dermatologist than your MD recommends, 2) use the best of the best products in the market, 3) if surgery is required such as diminishing scars (then do so), 4) it's proven (common sense) eating fresh fruits/vegetables, drinking plenty of water helps. I think it's pointless to use Neutrogena face wash, I only use water to rinse my face with a clean towel. I may get pimples on my back if I go for massages for example and what I realized in the past is 1) if the massage therapist is using their own oils I may break out (not bad, but I will get a few plus a red rash type reaction).
So what I did was, I changed my habits, I drank more water, I ate healthier, I took more showers (especially after back massages) and I used non-oily lotions that may harm my skin, and those worked for me. I do think that as we get older, it may just be a hormonal thing where the skin reacts differently unfortunately but I think once we get older it will clear up because just like the phase of us not having acne as kids, I would assume (I am no doctor) that as we put our teens, 20s behind us, the skin will clear up in our 30s, 40s, 50s, and etc.

This is a tough one, as I’ve had bad (not severe) acne throughout some of my teens & still expected to be liked by girls for being a young gentleman, but wasn’t 😔 In my honest opinion, I know a few girls I’ve liked with SOME acne such as a few red pimples under their make-up, but SEVERE acne may just be a little too much for me to be attracted to.
But I’ll give a word of advice: see a dermatologist, take whatever he/she gives you and use as long as required. In my case, it worked a treat after some months (in case you don’t see much improvement after only a few days). However, I was left with some boxcar acne scars on both cheeks which I didn’t feel too self-conscious of until a few years later. It subtly appeared to smooth itself out after using products such as acne scar removal cream, as well as moisturizer & microdermabrasion exfoliator twice weekly (preferably before bed).
Hope this helps. If that’s you in the photo, then you are pretty... just the acne may grab unwanted attention. But if you follow the advice I gave, boys will want to date you and see how lovely you naturally are under all that. Good luck!

Yeah, totally. If the person was confident enough to not let the acne control them, it's a none issues.
Most of us have something we aren't exactly all comfortable with or wish we could change, and people still love us for it no matter what.
In the end, if I'm attracted to them I'm attracted to them and won't let some acne stop me from expressing that.

I've never seen acne anywhere near as bad as pictured above. That said, if she is compatible with me, I'd still date her. If she was only so-so in terms of being compatible, I would not date her.
Sure, but I would want to focus on helping them heal and getting healthy as that would be my primary focus. That type of acne are signs of parasite infestation and that person needs to clean out and eat healthier. Stay away from conventional products and go organic all the way. The diet needs to change.

I've seen worse.. looks rate painful though and I truly feel sorry for whoever that is in the photo. I seen a guy around the university who just has it all over his face on every part..
If that is you in the photo.. well it looks like you, or they, have a pretty serious condition and it may be worth considering the possibility that the root cause may be dietary or digestive health related as skin problems so often are I believe. Again if it is you I wish you well in overcoming it. It is possible.
Another thought. I was wondering about the piercings. Googled "can piercings cause severe acne". Not finding anything definitive as such, but a couple forums on acne. org where people are discussing this - saying that if the piercing isn't cleaned properly as it heals, or if the piercing isn't done properly there could be a resulting infection which causes skin problems, also a few people mentioning that certain metals can cause a bad reaction in certain people, one where someone mentions that nickel might cause hormonal imbalance in some people. I don't have the time to read thoroughly, stayed up wayyyyy too late, but here are the links anyway, may be worth taking into consideration..

I used to have very bad acne where it covered my face for years. I kept fighting for my own treatment. All the common things didn’t work. Then I found a certain oil that got rid of my acne in a month. I cried.
So, yeah I would date someone with acne because I understand what it’s like and I see past it. However, I am not obsessed with skin and I’d offer to help and hope they’d want help for the acne and they aren’t just like whatever about it (because there is an answer for everyone uniquely!) and I’d wanna help.
I don't know if I would in general, but if it is this bad, sorry, but no. This is not "severe" acne. This is the worst case of acne I have ever seen.
The fact that she looks like this shows that this is not only an inherited condition - she has problems with personal hygiene, and she should have visited a doctor LONG ago.
People who say things like "poor personal hygiene" when there's severe acne know absolutely nothing. This is way more complex than that.

2) I actually have a beard. If you knew anything about beards, you'd know that they require constant washing as well as an idea amount of natural oil from your skin to avoid itching. This means that I can't wash my face with soap or other bodywashes, so yes, I wash my face 3-4 times a day thoroughly, ideally with hot water. I don't use products - I could, but I am not a big fan of them. Even so, you can see that I take quite good care of my face hygiene.

That's funny, because that's literally the first thing a dermatologist tells you. That no amount of benzoyl peroxide you smear on your face, it's worth jackshit if you eat potato chips all day, and don't wash your face with the appropriate cleaning implements (pH neutral soap, disinfectant, etc).

I never said it does. Just that for cystic acne to become THIS bad, you have to ignore preventative measures like facial hygiene, and actively avoid treatmeants.

I agree, it sucks for some people, especially since even after surgery the skin doesn't retain its tightness. I am sympathetic towards people like that. But generally speakign surgery, while costly, CAN help. Usually people who have cystic acne and are more well off (at least middle-class) can afford treatment, they just don't care enough about it. A friend of mine is like that. I told him many times - you want to get rid of it, visit a doctor. He's just stubborn and lazy. At that point, i give up.
Well I did have acne, it was mostly deep cystic acne, not that noticeable but very painful, and I tried everything and even going to the doctor didn’t help the first time. The second time they decided to give me a triple treatment all at once, antibiotics, the pill and benzolperoxide, and that helped a lot, but it is still not completely gone.
I also have two friends with very bad acne, far more noticeable than mine. One of them took about the heaviest medicine there is, and his acne is gone now. The other still has it and it damages his self esteem like crazy.
Okay I all ready posted in response to someone else saying that I've never had acne at all and throughout my life I have often gone days without washing.
I just had another thought though.. check this - in WWII right, and shit loads of other wars particularly living rough in the wilderness during combat without access to regular washing.. were there epidemics of severe acne amongst the soldiers? .. well, it's a rhetorical question. The answer is no.

1) I didn't bad hygiene causes cystic acne, I said it can be made worse by it.
2) In that past if you had cystic acne, they probably didn't treat it in the first place, or just rubbed alcohol on it, and you'd be "blessed" with gigantic scars on your face for the rest of your life. It's not that it didn't happen, it's that nobody really gave a shit, because acne was really low on the list of priorities, besides famines, wars, plagues, and just putting food on the table.
3) When I say "proper hygiene", you have to understand that all the mud and dirt in the past were less harmful for your skin than a single time you washed your face with soap. Soap removes your skin's protective oily coating, forcing it to excrete additional oil, which increase the chance of stuffed pores.
Additionally women also tend to wear makeup, a rarity in history. Makeup is the best catalyst for acne out there - it creates the perfect climate for it.
Of course you can play the moral superiority game, but the fact is that I am right.
@janna_jcb Fits the description of rarity I think, especially considering it was worn by certain social classes, in certain cultures, on an occasional basis. Meanwhile nowadays you'd struggle to find a woman on the streets who ISN'T caked in makeup.
Ancient egyptian pharaohs using wigs and charcoal eyeshadow every once in a while, and every girl on the streets having full makeup on with colored hair, fake lashes, lipstick and whatnot is really not even comparable. It would be intellectually dishonest to compare them.
It might be like that in America, but in Europe most women just wear mascara and eyeliner or lipstick. It is really rare to find a girl who wears foundation on the daily. And acne rates are just as high here as in America. Makeup only causes acne when you don’t take it off properly.

I highly doubt that there is actual, accurate data on the prevalence of acne, simply because most people don't necessarily seek medical help with it, as medication can be bought prescription free.

Yes, and my otorhinolaryngologist (say THAT 5 times in a row!) told me I should use a corticosteroid nose spray for my infected sinuses. Used it for 2 months. It was a knockout drug, probably wreaked havoc in my body. Well, turned out it was a false diagnosis, I just had a nose spray addiction.
I talked with a friend who takes anabolic steroids. He asked for advice on dosages from his GP, and the GP told him dosages that could literally kill a man.
Believe it or not, doctors make mistakes. They are very often stupid. Hell, there are several topics on Reddit about doctors fucking things up majorly:

Not saying your doctor was wrong... but on one hand you have a doctor... on the other hand you have concrete hard evidence which shows that makeup, and especially foundation can make acne a lot worse.

I had mild to medium acne in puberty, but it stopped for me quickly. No need to be jealous, the reason it stopped is because I was so cardinally underweight (anorexia) that my pores literally didn't produce enough oils to stuff my pores. My skin was often dry as paper. It wasn't a part of my life I am proud of.
Nowadays if I stopped doing proper hygienic care, I would have a shitton of acne, especially since I've been eating a ton of fat lately (I am bulking, I need excess calories).
But I never had cystic acne, thankfully, just a mild predisposition for acne in general, especially because of my tendency to grow a beard (a beard can REALLY easy develop pimples and stuffed pores if a few strands of hair grow unevenly)

Example - male pattern baldness is genetic. You either have it, or you don't. But if you have it, and you take anabolic steroids, the balding will severely accelerate. Having male pattern baldness is not a choice. Taking steroids which accelerate balding IS.
Same with acne. It's your responsibility to treat and prevent your acne to the best of your ability.

I have seen severe acne on severe acne with keloid scarring. No one here is going to approach a person with severe acne. I'm sorry but I am just being real. I won't judge someone who has severe acne because I don't know if it's genetics+puberty. Or if they have an autoimmune disease that effects their skin, or if that person is depressed and started becoming unhygienic
I had pretty bad acne as a teenager, I went to a doctor and he redirected me to a skin specialist. The guy gave me a medicine called Roaccutane for about 6 months, it's a very heavy medicine, I had to take a blood sample every 2 weeks to be safe. No alcohol for about 6 months. This medicine is very very effective and worth a try. I did not get any new acne ever since.
I had pretty bad acne myself, which luckily was able to be treated with accutane. I would date a man with severe acne, too, and maybe I could impart some wisdom I’ve accumulated over the years for how to help keep the outbreaks and cysts at bay.

Try washing, acne is caused by dirt and oils getting into the pores of the skin. Wash and you won't have acne.
Wow... how's that for over simplifying. Comes off patronising as well. She doesn't look like an unhygienic person; she looks like a person who has acne. I have never had acne, and I sometimes go days without washing or showering.. because I'm a fuckin' eccentric, weird creative and shit and sometimes I go through phases where my self-care isn't too good, so yeah I don't wash so well sometimes. My skin has never suffered from this.
