How to show him I am working on myself and trying to be less insecure/needy?

I should first explain that I have been struggling with mental health problems for a few years now. I feel very insecure and have social anxiety, with panic attacks when I feel overwhelmed. I met this guy a while back and at first I could see he really liked me and I also liked him so we wanted to meet in real life. Then stuff happened in my private and work life and my mental health got worse and worse to the point where I was scared he didn't like me at all and was just playing with me. I had to cancel our meeting but I explained why, I told him about my struggles, and he seemed to understand. Still, because of my anxiety I thought he was never going to wait for me. And things just got worse and worse, I was very needy and very insecure. I didn't want to bother him or disappoint him but I think the fact I was worrying so much bothered him. My job was also making things worse, until a couple of months ago I decided to quit. It was just one decision but it changed a lot. I feel like I can see things clearly for the first time in years, and I am realizing my mistakes. I know my behavior made me look insecure and clingy. I want to change that. I want to be a better person. We decided to organize a new meeting, which made me very happy. But I can see things are not the same. He barely talks to me, even though he says he wants to meet. Is it possible for me to show him that I really do want to change? And how can I do that?
