Is this considered a scam?

So I recently downloaded this hookup app and someone reached out to me whos looking for a friends with benefits. She sent me this link to fill out some form for ID screening to verify that its safe, but im a bit suspicious. Its not like she's asking for money or to invest in bitcoin or anything. I feel naive when im using apps like these I don't know. What are your thoughts?
