It looks as if the friendship has taken a beating. I don't know the reasons for it but if you were the cause for this incident, then you may as well consider the friendship as terminated.
A real friendship can take some ups and downs but when the limit is reached, then all the apologies are useless because it showed the true character of the person breaching the friendship. It is just like cheating or addiction. If the incentive to do it over again is high enough, it will happen. Guaranteed.
Your only option here is just to let her go and give her the space she needs. You may address her in a few months again with a regular "hi, how have you been doing" but limit it to that and don't start waking up bad memories.
