What I could say is that thick/bulky lips (even if many people tout them as attractive) are usually a sign of some oral irritant being applied or consumed. I consider only thin lips normal (and attractive).

Genetics can code for many different shapes and sizes of lips. Larger lips are not always due to an irritant or inflammation, and are certainly not unattractive to all women. Every girl has their own preference.

We have no evidence to conclude that his lip size is due to irritation. If it is not, and this is his natural lip size, then it is due to genetics. Every feature on our body is coded for by genetics. How else would lip shape and size even come to be? Genetics are the instruction manual for our body to develop in a certain way.
