Only once. I met a woman online and we agreed to meet at a Chinese restaurant.

I have had a few first dates for which that would have been justified but I’ve never done it.
Yes. First was one that walked in with freddy kruegar nails, more make up than a vegas show girl and had an entitled attitude from hell and talked to everyone around like she was the queen and demanded servants.
Stood up and walked out with not one word.
Second was someone who did nothing but surf her phone. I simple told her "I am sorry, I wasn't aware you were in a relationship already. i hope you and your phone are happy together" and left.
There have been others mostly based around poor personalities or not presenting who they really were when we met.

Ugh, I was on a date once where the guy snapped his fingers at the waiters like they were his dogs. When I called him on it after the third time he did this, he said, "what, they are working for us." RED F'in FLAG for me. Never saw him again and made sure to add an extra tip for the waiters.

Yes! Take a look at the comments: someone was nearly drugged, extremely rude dates, someone met up with a freaky liar, one caught a date doing drugs...I mean you are meeting up with a total stranger most times, that you know nothing about. Agreeing to a date doesn't mean you have to stay especially if you are in danger or someone is just being extremely obnoxious. Clearly your date probably experienced being stood up and didn't want to go through that again.
I have never, but if they came late or lied about their appearance before we met up, I would have no issue getting up and leaving like in @AviatorTom 's story
