What's wrong with my brother?

He's turning 27 in March and still no girlfriend. He has no friends from what I know, he spends his money on perfume and clothes, expensive food etc and can't drive.

No girlfriend but maybe he has FWBs. No driving is common nowadays if you don't feel the need to own a car. Spending on expensive dinners and fashion may be his joys in life. I don't know...

I think the main problem is that he has no or just few friends and copes with spending money over having no social life. Having no social life may make him look weird in the eyes of women and that makes him harder to get a woman fall for him.

You said he is a carer. My friend did that, and she devoted her life to it. They don't have time to socialise, so this is his way of functioning normally, I guess. Maybe he has anxiety etc.
