I have been dating a guy for three months everything was great but he ghosted me out of nowhere

I checked on him if he is okay and fine and he said yes all is good. So I believe we are over

my question is shall i leave the situation as is or text him and make it clear that we are over?

That was the main issue in the first place that we don't go out often and he cancel a lot
