He was never fully over her. Good rule of thumb is to never date the guy that is less than 6 months out of his last relationship. Science says it actually takes them longer to get over it than a woman because women grieve first and men go into denial first and grieve second.
During the denial period men will usually either do a bunch of silly or reckless things with friends to try to feel like their life is more exciting with her gone. Alternatively they will immediately look for her replacement. After the newness wears off a little he will start to remember and miss her because he did not confront his feelings yet.

Very possible. A lot of the men I used to meet when using dating apps were secretly still in relationships and trying to cheat. I had an ex that made a tinder behind my back, and I refused to be that naive other girl, so I would do a bit of research via reverse image search, etc, just in case. Some people have no shame
