I stopped having casual relationships a few decades ago. Calling a girl my “lover” is a way of saying “This is the girl who has sex with me.” Why do i need to say that to anyone? Our sexual relationship should not be a topic for public consideration.

Replying to your update; I didn't know you MUST have sex with your S/O if you want to give them a sexy nickname. Seems more like a title or an achievement rather than a nickname in that sense.

And to answer your question, no, I've never called my girlfriends "lover". I have called her love a few times, if that's what you meant

We use the same word "cariad" for lover/boyfriend/girlfriend regardless of whether or not your intimate. Its derivied from caru= to love and probably comes the same root word the Latin carus (dear or beloved). It can also be used as a girl's name

I think the meaning is innocent and straight forward enough. A lover would be someone you're in love with. Someone you share intimacy with. This is probably an interesting turn of the English language, but I feel like there's an additional meaning that's kind of taboo or illicit. I might use "my lover" to describe a mistress, but I don't even see myself doing that. Maybe I would use it to describe someone else's mistress.
