What is the most important thing in a first impression?

Be well spoken. I don't want answers to my questions to begin with, "Like, you know." In addition, do not use of pseudo-words such as wanna or gonna. Turn off your phone. Keep your jewelry modest. I don't want to be distracted by oversized earrings... or ANY size of nose ring!
If you are applying for an office job, arrive dressed as you would dress if you already had the job. This allows me to build a picture of you already working for me. If you don't have a specific job skill, simply say so and then focus on what you can do well. Tell me about your work astonishments- as an individual and as part of a team.
At the conclusion of the interview, I will ask you if you have anything you'd like to add. Please add something like how this company seems like a wonderful place to work, and how well you feel that you will fit in.
One last thing: if you walk into my office and try to assert your dominance (as miss @Feminist_Power suggests) your interview will be extremely short. I've worked for and with women during my entire working career. I know that women are competent. I do not need to be, nor will I be, lectured.

It depends on the context. You probably don't want to be making the same kind of first impression on a blind date as you would in a job interview, for example.
Make sure you have a very neat appearance. Don't dress like you're going to a wedding, but wear nice clothes. Smile. Be a good listener when they are speaking to you. Don't chew gum. Make sure your fingernails are kept trimmed and clean. I don't know why, but they will probably look at your hands for cleanliness. If you drove a car to the interview, make sure it is as clean as can be. Some interviewer's might walk you to your car to see what kind of car you have and if it is kept nice. If you are not sure where the place where the interview will be, do a drive by the night or day before so you won't be lost and you can be on time. Try and find out some information about the company so you'll have some questions to ask. Make sure you don't have bad breath. Bring your best attitude with you. Don't come off with some power attitude. Act like you'll respectively fit in their compay. They don't like people with a bad attitude, cause they don't need any employees that are going to start a bunch of crap with everyone. They aren't paying an employee to do that. Say please and thank you when appropriate. Smile, smile, smile 🙂
For me it is all about personality. You can be cute, rich, well spoken or whatever else, but if your personality sucks in just never gonna be interested in a second impression.

Physical appearance, people are obsessed with that and build entire perception around your personality on hand of your look. I don't complain, I share my observations in this case.
Assert your dominance as a woman. Men need to know that we built the world for them. Women made buildings and bridges, and cars and airplanes, and the internet.
