What are some signs of a toxic workplace?

I worked at a really toxic place. I guess I should have known since 90 percent of the people were there less than six months. I always felt like I was being watched and they never gave me much to do. My boss would schedule a meeting with me and then not show up. I would sit in an empty conference room by myself for an hour. It happened twice with no explanation. I was put in charge of a group of engineers but was not allowed to ask them the status of what they were working on. People would disappear on a regular basis and nobody said anything about it. The place was really toxic.

Nothing is ever done to improve the bad issues. Also you'll notice people quitting with little to no notice. No one wants to stay.
