I’ve always been self conscious about my looks…. And it seems like when I fall out with people they call me ugly… (one friend did in adulthood) it hurt.. I am trying to get plastic surgery

No it means you have low self esteem about yourself. , don’t change who you are to appease anyone , not everyone is going to think your ugly and not everyone is going to find you beautiful. Beauty is in the eye of the beholder , so just because someone doesn’t find you attractive, it doesn’t mean everyone else is going to think the same.
