Normal question to ask, especially with all the societal pressure to look attractive, especially with swim suit or beach season coming up and the ads constantly spamming.

There's this pressure to look 'naturally' beautiful, and if you're willing to pay for it, there are many cosmetic procedures can help you get there, it just depends on how much "risk" you're willing to tolerate.

A person can talk about a procedure and still decide not to do it. Sometimes it's also just a litmus question if you'd be supportive if they did do it.

My own grandmother asked me if I wanted plastic surgery on my nose when I was in junior high school.
My nose didn't need plastic surgery. It's a perfectly fine nose.

But she really hurt my feelings.

No, I think she is asking for herself. She wants to know if you would see it as normal if she gets one, I guess. Or she is asking for your opinion on people who do get plastic surgery in general.

Depends why she asked. She might be planning some for herself and was only asking your preference without even thinking of any imperfections you may or may not have.

She’s always talking about looks. Specifically how good this one girl looks with plastic surgery (I think she’s butt ugly but my friend thinks she’s stunning). I never bring up plastic surgery first. She’s very insecure about her looks and expresses so much insecurity about them and I’m always hyping her up.

sounds like she's nearly convinced herself she needs it, and wants you to admit you'd get it too to make her feel better about her choice.
I'd mess with her mind: tell her the opposite of what she thinks: if she thinks her nose is too big, drop hints it's too small. If she thinks her breasts are too small, tell her they're too big for her body, etc.
