I work at mcdonald's. There is this women who comes through the drive through sometimes. she's kind of rude. Never says thank you. I take an order out to the parking lot last night, and its this lady. She rolled down the window and a bunch of smoke comes out, and she had a cigarette in her hand. There was a child in the back in a car seat. I think there were other children in the car too. I almost said something but I didn't know how shed react, and even though id just be looking out for the children, I don't know if shed go to my boss with some made up story and get me fired.

It most likely wouldn't go well, as it's technically none of your concern.
Yeah, I understand you'd be looking out for the kids, but they're not your responsibility, nor do you have any means by which to enforce your request, so it would ultimately be meaningless.
All you'd be doing is creating conflict, and have no hope of getting her to stop, so the whole interaction would be a net negative.
Unless you want to get cussed out, beat up, or worse, mind your business. That is indeed a sad situation, but it's not your place to say something.
