Depends on where you are going and what you are comfortable wearing. You should give her an idea so she can dress similarly. If you are going to a fine restaurant, then be at least somewhat dressed -- a dress shirt and slacks. I hate seeing women nicely dressed out with a men in a T-shirt and jeans. Makes me think "What's wrong with him? Couldn't he make some effort?"

I think personally whatever she wants to. I don't really care. It also depends on where we are going.

Depending on how fancy the restaurant is, I'd say either somewhat dressed, or semi-formal. If the restaurant has low lighting, candles, flowers on the tables, and nice tablecloths, I'd say semi-formal. If not, go with the somewhat dressed option.
