Your breasts, unless you have genetic damage, are shaped like your mother, aunt, female cousins, grandmother and so on. They are genetically determined.
There are some breast-lifting exercises that can help strengthen the muscles behind the breasts, but other than those exercises, only supportive, specialty bras can help with breast lifting if you have extremely large or sagging breasts.
The other option is plastic surgery to make extremely large, sagging, genetically misshapened or damaged breasts appear more normal in size and shape.
There are many types and shapes of breasts that are normal. Don't alter yours unless they are outside the norm, which features a broad range of sizes and shapes, and certainly not before you have children, which will alter the size and shape of your breasts due to milk production and/or breastfeeding.
Lastly, your age will affect your breast shape as gravity affects this fatty tissue. Weight fluctuations do too, as stretching the skin means your skin will be less supportive. Again, good support bras will ease this issue.
Plastic surgery is the only solution to prevent shoulder scarring and back pain due to huge breasts. This is the only plastic surgery that will be paid for by health insurance.

What's your current shape and what do you want it to be? I don't think there's MUCH you can do, but if you tone the muscles in your chest, improve your back strength (and posture) and if your weight changes, all of those things can effect your boob shape.
