Because it arose around central Europe, probably Ukraine/Russia then spread west and north with migration and not south into Africa.

Probably due to the regions ethnicities adapted to. Africa and other equatorial countries have higher melanin due to more powerful sunlight. Northern regions don't and therefore don't need the malanin.

I think because blue is a recessive gene and brown is a dominant gene but I could be wrong it was a long time ago when I was in highschool
