Only once at a water park, and I didn't even notice til my friend pointed it out. The lifeguard lady didn't warn me :(

Other friends have had their top fall from after jumping in the pool but never have I had a problem at the beach or seen it

From my experiences yes, I've had problems losing my top, but this is typically not a problem for me due to the fact that I rarely go to the beach or swim.
