Im not talking about your just showing a lot of cleavage but like your nipple actually showing. I had that situation today when i was hanging out with a friend on campus. she was wearing a low cut dress and her bra was to loose so if you were taller then her you could see it all at some points. I told her but at first she thought i was talking about just cleavage so she didn't care. when she found out i meant everything she was kinda shocked. Did i do the right thing or should i just have left her ignorant.

Well, you do need to have some tact and common sense about it.
Yelling, "I can see your nipple!" in a crowded room would embarrass her. Saying, "Hey, I need to tell you something important." and telling her discretely is much better.
