This is not one I would make too much of. Your friend was speaking off the cuff and he probably gave what he said far less thought when he said it than you have already spent writing about it.
In general, he was just remarking about the easy way that you and your friend get along. Stereotypically - and it IS a strereoptype - you see that kind of easy going relationship more in siblings than in bf/gf relationships. The latter of which are - again stereotypically - more complicated because of the insecurities and complexities that we all bring to our relationships.
As to the sex part - that probably did not even occur to your friend when he made the remark.
Don't overthink this - and be happy you have a friendship with someone, of either gender, with whom you have such a good relationship. That is something worth having.

He's making a joke in poor taste. "Incestuous" is usually a slang term that means "so close it's mildly creepy." I've met a few friend duos who I would call "incestuous."
Why don't you ask the guy that said it?
He is the only one who truly knows what he meant by that.Sounds like a wannabe funny statement... It must mean something like "i care her like she's my sister but also i wanna do her" that kinda thing.
