Shrug and mentally move on. Clothes are just clothes; it's not like they actually change anything about you. And from what I know of crossdressers, most of them aren't even gay, let alone scary psycho serial killers (which has little if anything to do with being gay, it's just that the latter is more mainstream, more common, and usually the first thing a crossdresser is accused of being).

Also: context. WHY he's doing it might be important. "I like the way it feels/makes me feel"? "My girl/boyfriend likes it"? "I want to challenge social norms/freak the squares?" "I lost a bet/it's part of a joke/prank"? "It was the only thing clean/Costco had a sale on virgules"? Those are fine. "The Devil told me I had to wear this while I sacrificed my victims" is a different matter.
